Seeking an experienced Level 3 Social Worker in Rehabilitation, to join our client based in Canberra. Working on a permanent fulltime basis across a range of Hospital and Community settings.
We are seeking an experienced Level 3 Social Worker in Rehabilitation for a large tertiary service based in Canberra, joining on a permanent fulltime basis. Working as part of a multidisciplinary team in a range of hospital and community settings, providing assessment and therapeutic intervention for a range of client populations throughout their patient journey.
The Rehabilitation Aged and Community Services team (RACS) provides integrated and effective services in the areas of rehabilitation, aged care, and community care in a broader range of sites throughout the ACT, including Hospitals, community health centres and the homes of clients. This includes health care and support for people with acute, post-acute and long-term illnesses.Services include facilitation of access to resources, responding to crisis, counselling, palliative care and bereavement issues, staff consultation and in-services.
Social workers provide assessment and therapeutic intervention for a range of client populations throughout their patient journey. The patients we see present with a range of psychosocial issues that impact their health circumstances across the lifespan including ante-natal care, newly acquired and chronic health conditions, medical and surgical treatments, rehabilitation, and ageing.
Key Responsibilities
Under limited direction of the Rehabilitation Aged Care and Community Services (RACS) Social Work Manager you will perform the Social Worker role under limited supervision and understand the issues related to health and wellbeing and the impact on the person and their family/carer, including adjustment to change in their health. The Social Worker will promote improved client outcomes through working in collaboration with the multidisciplinary team to provide high quality clinical services across a range of service speciality areas. You will:
- Provide a high level of appropriate skilled clinical assessment and psychosocial interventions across a range of clinical areas, including individual and group work to consumers and carers
- Exercise independent professional judgment in solving problems and managing cases where principles, procedures and techniques require expansion, adaptation, or modification
- Liaise with patients, carers, and service providers, to enhance and improve clinical outcomes and develop partnerships for health promotion and other service activities identified in the business plan
- Contribute to the supervision and education of staff and students
- Participate in supervision, continuing professional development and performance management
- Contribute to the implementation of clinical governance activity, quality improvement projects, research programs and health promotion in areas relevant to service
- Actively participate in team meetings, complete clinical and administrative data collection and case records to a consistent high standard
- Practice in accordance with the professional body’s code of conduct, practice standards and organisational guidelines and legislative requirements
- Undertaking other duties as directed, within the approved scope of clinical practice ensuring the delivery of high-quality person and family centred, safe and high-quality patient care
About You
- Proven knowledge of and ability to provide a high level of skilled clinical psychosocial assessment, counselling, and complex interventions, which is responsive to multi-faceted patient needs
- Demonstrated experience working collaboratively within a multidisciplinary team
- Strong level of communication skills and ability to establish and maintain effective working relationships with internal and external service providers and stakeholders
- Demonstrated critical self-reflection and active participation in professional supervision and provide clinical supervision to professional staff and students
- Experience in undertaking innovative quality improvement activities through policy review, quality improvement processes and research to develop evidence-based practice
- Understanding of, and adherence to, safety and quality standards, work, health and safety (WH&S) and the positive patient experience.
- Displays behaviour consistent with CHS’s values of reliable, progressive, respectful and kind
- Relevant Social Work degree/qualifications and a minimum of 3 years’ experience working professionally in hospital or health is preferred
- Be registered (or eligible for registration) with the Australian Association of Social Workers (AASW)
- The successful applicant will need to be available for occasional weekend and after-hours work, with access to flex time
Annual Remuneration package: $105,900 - $119,774 (Plus 11.5% Super)
