Role: Band 5 Community Nurse / District Nurse

Pay rates: £19.32 - £42.19 per hour.

Are you a motivated District Nurse looking for change? 

Medacs Healthcare are looking for Community Nurses / District Nurses to join our team to provide care for patients in the community in the Macclesfield area, via East Cheshire NHS Trust.

Main Duties of the Job

Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge to home.Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient’s privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment. 

Implement / refer on to the relevant services as appropriate. 

Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.

Essential Qualifications Required

You must have a valid NMC registration with at least 6 months experience within the NHS within a Community Nurse / District Nurse role and right to work in the UK.
