The Role

• Work within the community servicing older adults in the My Aged Care and CHSP funding stream, with little to no NDIS
• Fully mobile, have your day planned out and work within 30 minutes of your home
• Enjoy a work from home day during the week to allow time for admin and report writing
• Collaborate with other clinicians across a multidisciplinary team
