About the Roles

You will contribute to patient care across various inpatient settings, including:

• Aged Care Transition Services: Supporting older adults transitioning from rehabilitation or medical care to residential aged care within a structured inpatient setting.
• Inpatient Rehabilitation: Working with adult patients recovering from stroke, amputation, or general reconditioning needs, focusing on functional improvement and independence.

Key Responsibilities

• Conduct comprehensive assessments to identify patients' occupational needs.
• Develop and implement individualised care plans to support recovery or transition.
• Collaborate with multidisciplinary teams to deliver integrated care.
• Provide patient education and support for improved outcomes.
• Document and report progress in line with hospital standards.
