About the Role

We are looking for dedicated Senior Support Coordinators to assist participants in navigating the NDIS system and accessing appropriate supports.
Multiple positions are available, focusing on high-level Support Coordination for individuals with complex needs. This is an opportunity to make a meaningful impact while working within a supportive team environment.
Key Responsibilities

• Implement and manage NDIS plans, connecting participants with appropriate services.
• Provide tailored support coordination, including high-level case management for complex needs.
• Monitor progress, review supports, and provide recommendations for adjustments.
• Liaise with families, service providers, and stakeholders to ensure a holistic approach to care.
• Assist participants in navigating the NDIS system, resolving issues, and advocating for their needs.
• Promote capacity-building and independence for individuals.
• Maintain compliance with reporting, documentation, and regulatory requirements.

Skills and Experience

• Relevant qualifications in social work, psychology, or community/disability services.
• Experience in disability, social work, or community services.
• Strong understanding of NDIS plans, frameworks, and policies.
• Excellent communication, problem-solving, and advocacy skills.
• Ability to handle high-pressure situations and work both independently and collaboratively.
• Strong organisational and time-management abilities.
• Commitment to upholding ethical standards and professional boundaries.
• Proven ability to meet billable and KPI targets.
