The Role:


The Coordinator position offers a unique opportunity to lead a team that provides high-quality care and support to individuals with disabilities. Working as part of a client-focused, collaborative team, you will directly engage with clients, their families, and allied health professionals to deliver personalised support services.

This role is ideal for someone passionate about client-centered care, team leadership, and building strong relationships within the community. Responsibilities will vary, with tasks ranging from managing small caseloads in home settings to overseeing supported independent living (SIL) arrangements.

Key Responsibilities:

  • Team Leadership & Accommodation Management:
  • Lead and mentor support teams on a daily basis.
  • Build and maintain strong relationships with clients' families, allied health professionals, and other key stakeholders.
  • Manage the financial aspects of client care, including reporting and budgeting.
  • Track client health, wellbeing, and appointments, ensuring appropriate follow-up.
  • Ensure all required client records are up to date and compliant.
  • Oversee the management of physical premises, ensuring proper functioning of accommodation.

Direct Support Work:

  • Provide hands-on support and care to clients according to individual plans.
  • Participate in shift work as required to ensure continuity of care.

Rostering & Recruitment:

  • Recruit and onboard new support staff, ensuring all shifts are adequately covered.
  • Review and approve client rosters, managing scheduling conflicts and cancellations.
  • Conduct interviews and assess candidates' suitability for roles within the team.

Team Onboarding & Training:

  • Facilitate training sessions for new team members, ensuring they are well-prepared to meet client needs.
  • Organize onboarding activities, such as shadow shifts, to ensure staff understand organizational policies and procedures.
  • Hold regular team meetings to provide ongoing training, mentorship, and professional development.

Client Relationship Development & Management:

  • Develop a deep understanding of each client's routines, preferences, and goals.
  • Proactively identify and resolve concerns to maintain the wellbeing of clients.
  • Maintain regular communication with clients, families, and support teams to ensure high standards of care.

Compliance & Safety:

  • Assist in managing health, safety, and wellbeing requirements for both clients and staff.
  • Lead safety initiatives and ensure adherence to safety protocols.
  • Participate in incident reporting and manage necessary follow-ups.

Other Duties:

  • Participate in a rotating on-call roster to ensure 24/7 support coverage.
  • Be flexible and ready to take on additional direct support shifts or other duties as needed.

To Be Successful in This Role:

  • Experience in disability, health, or community services, with a focus on direct client support and team coordination.
  • Proven experience with rostering, scheduling, and using record management systems.
  • A passion for client-centered care and working in a collaborative, team-oriented environment.
  • Strong communication skills, including both written and verbal communication with clients, families, and support teams.
  • Comfort and proficiency in using technology, including cloud-based apps for rostering, incident reporting, and communication.
  • Leadership experience with the ability to inspire, motivate, and guide a team.
  • Relevant qualifications or equivalent experience in community services, case management, or a related field.

Must be willing to work the occasional disability support worker shift. Additionally, this role will be all rounder position where you will be required to be responsible for rostering, recruitment/interviewing and managing a caseload of participants. There will be occasional requirements for travel. 

Additional Requirements:

  • Completion of the NDIS Worker Screening Check (mandatory prior to commencing).
  • Current First Aid and CPR certification.
  • A valid driver’s license and access to a vehicle (mileage reimbursement provided).
  • Must be based in the Geelong region
